Roster / Waiver / Game Report / Directory .... updated 3-13-08
Please NOTE that the above document is an Excel document- you must have Microsoft Excel to open document and to type in the fields. When the document is opened, there are 3 tabs in the lower bottom corner that read: Roster / Waiver Form / Game Report, select proper tab to print document. If you start with the roster tab and fill out all the necessary fields, those fields will populate all other tabs in document.

Registration
Please follow directions: 1. open document in the Adobe PDF Reader application, 2. type in your information on screen, 3. PRINT the form with your browser's print function, 4. sign and submit printed form via mail, fax or deliver it personally to MDSL HQ.
