RE: 2009 Registration Procedure

Friday - March 27, 2009 MDSL Player Registration Deadline

Forms and Fees 15 / team minimum

(All passcards must be returned to me)

Roster, waiver, game report, components, log and directory have been emailed to returning and new teams. If you have not received yours please notify me as soon as possible.

Please download these forms and save them. Returning teams must use this roster to make additions and deletions for the 2009 season and return to me electronically at: hlipka@wowway.com. or hlipka@comcast.net . Also, please submit a hard copy of the roster and copy of the components check list when you submit your registration. New teams must use the MDSL Roster, Waiver, Game Report, Component, Log and Directory found on our website detroitsoccer.com. Please do not FAX any forms to the office. I will not accept them.

All players must fill out an individual MDSL player registration form (completely and legible)

All players must submit legible copy of driver’s license (Please do not enlarge them)

All players must sign and date the waiver form prior to the beginning of the season.

New players, must submit 2 identical photographs of you alone sufficiently recent to be a good likeness of you (normally taken in the last 6 months) and 2 x 2 inches in size. Photographs must be clear front view, full face taken in normal street attire without a hat or dark glasses. Please print players name on back of pictures.

You may submit pictures on a CD. (Please identify all players)

All Coaches, managers and delegates will be issued ID Cards for the 2009 Spring Season

All Coaches Managers, Delegates must fill out a MANAGEMENT form (completely and legible)

All Coaches, Managers, Delegates must submit legible copy of driver’s license (Please do not enlarge them)

All Coaches, Managers, Delegates must submit 1 picture

All Coaches, Managers, Delegates must sign and date the waiver form prior to the beginning of the season.

Please fill out all information on the roster.

Dual registration – (playing for more than one team in MDSL or other League)

Players must fill out the Multiple Player Form and submit 1 picture

You can mail, email, or drop off registration at the MDSL Headquarters
